That may be a non-starter… Can’t bring up an external by holding ‘option’
on restart as (I assume) as something must be interfering with that key. Can’t start up the system as-is to use Startup Disk Manager as I can’t log in - whatever it is is buggering up my password.

    try an external keyboard.

Try more than one if the first doesn't work. I've had some trouble over
the years with Apple wired aluminium and the old white+clear ones
failing to work at Mac boot screens. Random USB PC keyboard always
    works.

Did you also do an SMC reset already? It's a bit voodoo (even more so
than clearing PRAM back in the day) and the Apple page doesn't suggest
any interaction with keyboards, but it's also never a bad thing to try. https://support.apple.com/en-au/HT201295

I am suspecting a physical issue though.

Well, I’ve got one of those old white+clear keyboards attached (wonder if
it came with my old G5 iMac?) and a USB mouse. I’ve ‘booted’ from an external (actually the old SSD from this laptop w/full Mojave install). Or rather I haven’t - it’s stuck on a black screen. Coincidently I booted from it a couple of weeks ago when checking all my old drives. It took several minutes to boot then, but now it’s been 10/15 minutes.

Anyway, the external keyboard works fine. The mouse doesn’t - looks like stuck/glitchy modifier keys maybe (although i noticed a couple of the
function keys playing up too). Would a key being pressed interrupt the boot from the external drive?

Aye, the SMC reset did nothing. Thanks for the suggestion though - I’d forgotten about that!

Could (like the mouse) some of those keys being glitchy affect the SMC
reset? I see the KB article you linked says ‘internal keyboard’, so I assume it won’t work with an external (although I found a terminal command
to - theoretically - disable the internal keyboard).

2012 MBP on Mojave. The keys seem to have a life of their own. Sometimes
caps are on all the time with the occasional letter(s) correctly showing in lowercase, sometimes can’t unselect a desktop folder - just click on another to also highlight it. Sometimes holding alt fixes that, sometimes not. Can’t select from a list - multiple selections highlight but can’t get
rid of highlights. Clicking once in a text document highlight the word or several words…

I’ve run DU & it says all is fine (and restarted a few times). Managed to do TM and CCC backups. Could it be a physical thing?

I had a similar issue years ago with the trackpad. Turned out to be a
swelling battery. Does the keyboard look a little deformed?

it's a complete waste of time.

Strangely, as I was told only yesterday when talking to Apple Support
staff by telephone, it is the ONLY non-Apple software they are allowed
    to use/recommend for combatting malware.

What is YOUR view of EtreCheck (from www.etresoft.com ) the most widely promoted 'tool' suggested on the Apple Support Communities (ASC) forums?

Yes and with a 10 year old laptop, that sadly must be a distinct
possibility :( Let’s hope not. Could be just gunge under the keys.

Battery seems fine. Gently brushed all the fluff/dust inside then gave it a good blowing (under the keys too)… No help. Tried another SMC reset - no good. Rebooted a couple of times and the final time it booted itself into
Safe Mode(!) Not that that helped. More weird randomness - now getting UI glitches on Safari (but only on the tab bar). Won’t recognize USB sticks or SD cards…

    Time to go shopping.

Has it been possible to perform a diagnostic test? D key held down on
    startup etc.

I suspect not, given the state of your kb but might work with an external
kb. Whether the results would be useful remains to be seen I guess!
